Necessary Forms


  • If you are over 18, you are required to have the following: Form A (online registration), B, E, F (fees) and G (Proof of Insurance).
  • If you are UNDER 18 with a parent or guardian you must have the following: Form A (online registration), Form B cosigned by the accompanying parent/guardian, E, F (fees), and G (Proof of Insurance).
  • If you are UNDER 18 without an accompanying parent or legal guardian you must have the following: Form A (online registration), B, C and D (Unaccompanyied minor forms with MUST BE NOTARIZED), E, F (fees) and G (Proof of Insurance)
  • YOU MUST BRING ALL FORMS WITH YOU TO RACE (besides the Event Entry form which is filled out on-line and kept with Registration Officials.) Fees are either paid online or payment is due at check-in.
