Minor Permission to Participate Form for May 31 and June 1, 2008

This form must be completed and presented to GSI officials prior to event.

Please read carefully.

**If the adult-in-charge of the minor is present at the event,
they should keep this form with them at all times during the event.**

Minors under the age of 18, when not accompanied by a parent or legal guardian, must have a notarized affidavit from the parent that states:

1. The child is participating with the permission of the parent.

2. That the non-present parent or guardian is aware that the child is participating in a Gravity Sports International downhill race event.

3. The name(s) of the person(s) accompanying the child.

GSI is asking all minors to have BOTH PARENTS sign two (2) permission forms, both of which MUST be notarized. If parents do not live together, parental consent from both parents is still necessary. If one parent is the sole custodial parent, there MUST be legal proof/documentation of this status.
